5 - Agent will be issued a writing number

After you complete your 2011 AHIP training you will still
be eligible to sell Bravo Health's 2010 products until the 2011 products are

In an effort to better serve you and provide the most timely commission
payments, I am requesting that you submit your
Highmark
business via this
excel spreadsheet by the 1st of every month. When submitting
your
January 1, 2012 business also include all of your January
effectives where a renewal commission is due to you. Continue to include those
renewals for every month thereafter.
